Position Summary:
The Care Coordination Medical Director is responsible for providing clinical leadership and oversight to the Care Coordination site and is accountable for a care management system that promotes access to appropriate healthcare and education for members and friendly service to all stakeholders of the healthcare delivery system.
 
Primary Responsibilities:
  • Helps to ensure that the Care Coordination program results in improved coordination of care and services for patients, thereby achieving established targets and goals.

  • Serves as a primary clinical educator and mentor for his/her Care Coordination Unit.

  • Develops and delivers clinical education programs that address the specific needs of the Care Coordination team.

  • Together with the Care Coordination team, identifies and, when appropriate, assists in initial investigation and triage of quality of care and patient safety issues.

  • Monitors site-specific Clinical Operations with the Care Coordination Team -  dash board data, identifying opportunities and assisting in the development of appropriate action plans.

  • Supports sales and marketing efforts through customer presentations related to Care Coordination process.

  • Monitors both the customer specific and product line reports, identifying changes in utilization and access patterns and developing targeted action plans.

  • Participates in the design and development of Care Coordination programs and national committees as directed by Care Coordination leadership.

Qualifications
  • Licensed physician; Board Certified in ABMS specialty

  • 5+ years clinical practice experience

  • Supervisory skills, including clinical mentoring and coaching expertise

 
Additional Assets Preferred:
  • Excellent presentation skills for both clinical and non-clinical audiences

  • Excellent interpersonal communication skills (telephonic and face to face)

  • Ability to establish rapport with physicians in peer-to-peer communications

  • Solid knowledge of managed care industry

  • Data analysis and interpretation experience and skills

  • Strong belief in Care Coordination philosophy

  • Creative problem solving skills

  • Strong team player and team building skills
