At Home Rehab is a locally owned, skilled homecare agency based in Grand Rapids, MI.  We have an open position for a per diem MSW to join our growing team.  If you desire flexibility with your schedule and enjoy working with clients in their own home, then this position may be right for you.  We are committed to helping clients achieve maximum independence and safety in their own homes.  We need an MSW who has experience working with the geriatric population and strong knowledge of the community resources available in the West Michigan area. 

Requirements:

Master's degree in Social Work

Minimum of 2 years of experience in acute care, SNF/rehab facility or homecare

Homecare experience preferred
