About the Job
The MMG Clinic Complex Case Manager functions as a member of the multidisciplinary team, focusing on the improvement of clinical outcomes and the quality of life for Meriter Medical Group (MMG) patients with complex medical and social issues. This position works collaboratively with physicians and clinic staff by providing a comprehensive, well-coordinated care plan that supports the continuum of quality care, with a focus on but not limited to identified PPIC members. This position is responsible for the early identification of patients with complex issues by use of risk screening criteria, developing a comprehensive care plan that represents the continuum of care for the patient, and provides personalized education to the patient and/or family that aide in living with the complex condition. Individualized care plans for each patient taking into consideration:
~Early identification of home care and alternative care needs ~Minimizing preventable hospitalizations and emergency room care ~Abating the progression of disease states ~Promoting health and recovery ~Preventing complications ~Ensuring that quality care is provided in the most cost effective manner.
Following the nursing process, this position assesses, plans, implements and evaluates the patient's ability to manage their medical and/or psychosocial conditions and meet goals. This position effectively communicates with the patient's MMG Primary Care Physician and appropriate additional healthcare providers to promote continuity of care and quality of life, while maintaining a cost effective care plan.
The ideal candidate will possess: ~BS in Nursing. ~Registered Nurse Licensing in Wisconsin required. ~Knowledge of payer related rules and regulations and reimbursement venues to include Medicare and Medicaid. ~Knowledge of developmental issues for population served. ~Knowledge of community resources. ~Minimum of 2-4 years of nursing experience. ~2 years experience in a clinic setting. ~Experience within an outpatient setting. ~Excellent organizational, interpersonal, time management and continuous process improvement skills. ~Project management skills with the ability to prioritize, problem solve and handle multiple tasks. ~Ability to maintain confidentiality. ~Ability to work independently with attention to detail. ~Strong interpersonal and communication skills (verbal and written) with the ability to work well within a clinical team setting (Physicians, nurses, ancillary staff). ~Ability to partner with patients to create a collaborative relationship focusing on utilization of patient strengths. ~Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. ~Effective customer service skills and orientation. ~Strong conflict resolution and independent decision-making ability. ~Knowledge of clinical guidelines (i.e., Wisconsin Collaborative). ~EPIC. ~MIDAS. ~Computer skills, including MS Office products.
Preferred (Education, Experience, Skills):
~Masters Degree. ~Certified Case Manager (either ACMA or CMSA). ~Minimum of 1 year of case management experience.
