Our vision is to provide the best patient care experience for every patient, every time, and to be the best place to work for all of our employees, volunteers and medical staff Join our team of excellent caregivers. * Care Manager, Clinical Social Worker The Care Manager, Clinical Social Worker is a resource for the patient's treatment team in assessing the patient's psychosocial needs during their hospital stay. Works collaboratively with the patient and their natural support network to develop plans for post-hospital care. Serves as an advocate for patients and families and as a resource and educator for hospital personnel and medical staff. Requires a graduate degree in Social Work with an active Vermont LICSW or equivalent. Five years experience in mental health counseling, including psychosocial and substance abuse assessments, development of treatment plans, and a working knowledge of family systems theory. Two years in a medical setting preferred. Knowledge of referral sources, insurance, and state agencies RNs with recent care management experience will also be given consideration.
