Nurse Case Manager for a downtown, Manhattan managed care organization. There is the option to earn the right to work from home after probationary period.
- Achieves optimal benefit and results from the comprehensive management of patients with chronic and/or catastrophic illness, patients who are frail elderly and/or patients whose illness is complicated by challenging psychosocial conditions.
- Identifies patients appropriate for case management early in their disease process, preferably at enrollment with Health Plan.
- Develops, facilitates and communicates a plan of care in partnership with the patient, his/her significant other, the primary and attending physicians and various providers.
- Optimizes both the quality of care and the quality of life for the patient.
- Minimizes risk for the patient, the employer and the Plan.
Requirements
- Graduate of an accredited 2 or 4 year nursing program (advanced degree preferred).
- Current license as a registered Nurse in New York State required.
- Certification as a Case Manager preferred.
- Minimum of 5 years of clinical experience in acute high-tech or home health setting.
- Prior case management and or managed care experience preferred.
- Process Oriented with Strong Communication, Computer and Organizational Skills.
Salary up to $77,000 with excellent benefits
