RN Care Manager - Hudson Valley area. Meet directly with members and providers in an office/home setting to formulate a care plan to meet the member’s needs.  Provide necessary medical and supportive services to achieve the highest quality outcome.  RN licensed in New York, 5 years experience working in home care, hospital and/or skilled nursing facility settings.  Spanish bi-lingual skills required.  Per Diem RN positions also available for both Albany and Ulster county areas – proficiency with the SAAM and/or OASIS assessment tools required along with excellent assessment skills.
