- Educate members on available tools
- Provide assistance and educate members on accessing and understanding cost and quality comparisons
- Coordinate care for members based on treatment plan
- Provide referrals to other Care Management and Disease Management programs
- Maintain telephonic communication with members to ensure member's needs and/or issues are addressed - Facilitate benefit savings for members by proactively educating member on benefit plan impacts and choices
- Provide emotional support to members during acute/chronic illness
- Link and connect members to community resources based on need
- Listen, ascertain customer needs, and determine appropriate action within defined scope of authority

 
Job Requirements:
- Current active Florida RN License
- 2+ years clinical nursing experience providing patient care
- Position is primarily in a call-center environment (that will require working on the phones up to 8 hours daily) but may also involve face-to-face consulting
- Will require flexible hours/shift that may change frequently depending on business need (requires availability between the hours of 7am-7pm)

Business Specific Criteria (preferred skills):
- Case management, disease management, health education or home health care experience
- Experience educating health insurance members on how to maximize the use of their benefits
- Experience working with community resources
- Bilingual (speaking, reading and writing English/Spanish fluently)
