Nurse Liaison - Boston Area Facility
About the Job
Job Purpose:
Enhances Continuity of patient care by providing liaison between assigned hospitals, physicians and home care agency
Qualifications:
* A current licensed Registered Nurse, Associates or Bachelors degree, in good standing in Massachusetts
* Familiarity in Case Management (preferred)
* Experience in marketing, related to home infusion therapy, nursing homes, home health services, medical sales or pharmaceutical sales
* Field experience a plus
* excellent customer service skills (verbal/written)
Duties:
* Transitions patients from hospital to home care services by establishing and maintaining relationships with referring hospitals
* Provides nursing care and a treatment of home infusion patients in accordance with the physician's plan of treatment and with company policies and procedures
* Ensures patient competence and compliance with all self-care, including procedures, re-educates as necessary and documents/communicates the need for re-education.
* Plans and implements educational in-serves for case managers, social workers and discharge planners.
* Makes direct calls on health professionals, including physicians, office nurses, home health nurses, staff nurses, discharge planners and social workers to educate them about services available to their patients.
* Works with marketing, nursing and pharmacy in development of new therapies
* Participates in Nursing On-Call program
* Continues to maximize opportunities in territory.
