Nurse
Practitioner or Clinical Nurse Specialist- Palliative Care

Palliative
Care and Hospice program currently seeks a visionary Nurse Practitioner or Clinical Nurse
Specialist to
join a growing team of individuals who are passionate about the continuum of
care from chronic disease management through end-of-life care.

The
Palliative Care Advance Practice Nurse will consult and partner with the
Palliative care team to direct and facilitate the care, support and education
of patients, families and the physicians who are treating patients that are
facing a life threatening illness.

Prescriptive authority preferred. This will
be a  position that involves,treatment and coordination of care between acute care
facilities and/or home services.

Nurse
Practitioner or Clinical Nurse Specialist or APN, required. National
certification required within 1 year of employment.

Minimum of 1-2 yrs as NP or
APN required.
