Position Details:
Work under direct supervision of a physician in accordance with state and federal guidelines and rules.
Provide primary and preventive medical care to meet the needs (physical, mental, and emotional) of patients under a limited scope of CP codes and within his/her skills and training.
Provides medical patient care to the scope and standards of practice established by a nationally recognized credentialing organization representing Nurse Practitioners.
Functions within the scope of practice of the state Nurse Practice Act and state Medical Practice Act.
Emphasize wellness, prevention, and early detection. Instruct and counsel patients and families regarding matters pertaining to their physical and mental health with regard to health promotion, diagnosis, prognosis, and management possibilities. Provide support and counseling for patients with chronic disease with regard to psychological, environmental and physical well-being.
Supervise support staff in delivery of care to patients.
Maintain universal precautions and infection control practices.
Provide smooth and timely flow of patients in accordance with the VA’s access standards and triage protocol.
Recognize and evaluate situations which call for the immediate attention of the physician. When necessary in these situations, initiate treatment procedures necessary for the life of the patient. Perform cardiopulmonary resuscitation (CPR) and assist during respiratory and cardiac arrest procedures.
Set up and operate therapeutic and emergency equipment, including monitor, EKG, oxygen, and portable suction.
Obtain patient history on new and follow‑up and record appropriate findings.
Record patient medical history, perform a physical examination, collect appropriate reports, and make an assessment and diagnosis there from and present pertinent data in a manner meaningful to the physician. Perform initial and periodic physical examinations within limits of privileges and record findings.
Provide treatment to patients within scope of license and privileges.
Demonstrates the knowledge of pharmacology.
Prescribe and administer medication within limits of privileges and state and federal regulations.
Perform a range of nursing procedures, including dressing changes, suture removal, oxygen administration, wound irrigations, catheterizations, and venipunctures.
Facilitate referral of patients to the appropriate specialists, health facilities, agencies, and resources of the community. Institute referral and coordinate patient services to promote continuous comprehensive care at the secondary, and tertiary levels. Recommends and follows-up on appropriate laboratory tests and specialized diagnostic procedures as deemed necessary.
Prepare, perform, and schedule patients for laboratory procedures by providing information and instruction.
Order or transmit orders for x-rays, laboratory studies, therapeutic diet, respiratory therapy, or other diagnostic studies necessary to arrive at a diagnosis for the patient. Perform and/or schedule special diagnostic tests or procedures at the direction of the PCP.
Document all pertinent patient information in the medical records and VISTA to demonstrate quality care delivery and promote continuity of care.
Maintain the VISTA clinic reminder tracking system current for each enrolled patient.
Assist in scheduling patients for initial and follow-up appointments in accordance with the VA’s access standards and the patient’s medical necessity.
Keep patients informed of their plan of care, including lab and diagnostic test results as needed.
Provide health educational materials and resources to patients and their families for informational purposes.
Responsible for the CBOC’s compliance with all federal, state, local, JCAHO, OSHA, VA, HNFS, and Subcontractor safety and operational regulations, directives, and standards.
Participate in the ongoing Performance Improvement Program between HNFS, its Subcontractor(s), and the VAMC.
Ensure that all required reports are completed in an accurate and complete fashion.
Participate in staff development programs.
Perform, under the PCP’s supervision, administrative and clerical duties:
Maintain clinical records; Schedule tests; Schedule return visits; Triage telephone calls; and coordinate other health services resources/referrals, institutional or community.
Perform other tasks delegated by the supervising physician, which are provided for by regulation or usual and customary practice which fall within the area of competence of the supervising physician, and the skill and training of the non-physician provider.
Maintain confidentiality of all information and support patients’ privacy, rights, and safety.
Assist the RN Manager in the management/coordination of the CBOC’s day-to-day activity.
Ensure all patient information on encounter forms are completed an accurate.
Act as a liaison between HNFS and the VAMC.
Perform other work‑related duties as assigned.
Experience/Qualifications Needed:
Graduate of an accredited school of nursing and a graduate of an accredited program for nurse practitioners, including preceptorship.
Certification by the applicable professional organization.
Minimum of one (1) year experience within the past three (3) years as a provider in a related area of patient care. Current state license. Must comply with all state requirements and regulations.
Obtain BLS certification prior to performing work.
Knowledge of and ability to apply professional nursing principles, procedures, and techniques.
Ability to assess patient’s condition, recognizes adverse signs and symptoms, and reacts swiftly in emergency situations.
Ability to set realistic, measurable education goals for patients, taking into consideration patient/family needs, level of understanding, and available local resources.
Ability to develop and provide individualized and group counseling, guidance, and health instruction with a focus on disease prevention and health maintenance.
Ability to teach patients and/or families proper home health care activities, such as giving injections, taking blood pressures, changing wound dressings, etc.
Knowledge of pharmacological agents used in treatment, including desired effects, side effects, complications, and patient usage considerations.
Knowledge of the basic concepts of customer service techniques related of age-specific population.
Demonstrated effective verbal and written communication skills along with proper telephone etiquette.
Working knowledge of Microsoft Office Software and basic computer maneuverability.

Monday through Friday, day shift.
