Nurse Practitioner - Transitional Care Program

In this role, you will provide direct care, perform home visits and provide direct leadership to the “new” Transitional Care Program to reduce readmissions to the Hospital. This is a Full Time Grant Position.


To qualify, you must have a NJ RN license and APN licenses, at least 3 years acute care experience and identified leadership experience. Graduate of an NP Program, with Adult Health/Acute Care/Family/NP certification is required. Bilingual Spanish is a PLUS.
