Program: Hospital-Based Outpatient Program For Senior Adults
Location: Presbyterian Hospital - Commerce, Texas
- Responsible for the overall coordination, administration and management of psychiatric services.
- Ensures high quality, high utilization and adherence to both Diamond Healthcare and Hospital Policies and Procedures and Plans.
- Communicates with corporate office, hospital Liaison, referral sources and medical staff to ensure the coordination of efforts in achieving organizational goals and objectives.
- Coordinates Performance Improvement and Survey activities (JCAHO, Medicaid, licensing and Medicare).
- Ensures prompt response to referrals and completes initial assessments on prospective patients.
- Serves as liaison between physician and other members of the treatment team.
- Ensures that all interventions (i.e., individual, group and family therapy) are performed and documented according to professional clinical standards.
- Provides meaningful input into treatment goals, objectives, and design of the treatment program.
- Identifies and plans strategies to reach existing referral markets and potential new markets. Supervises planning and implementation of outreach activities such as community education, special events, speakers bureau, exhibits, etc.
- Facilitates the resolution of questions/problems by patients, families, companies and/or community organizations regarding program services.
