RN Care Coordinator


The RN Care Coordinator works with an interdisciplinary team including physicians, nurses, therapists, and other healthcare providers in order to develop appropriate plans of care for elderly and disabled patients receiving home health care.


 


Qualifications: 


·        Graduate of an approved Professional Nursing Program.


·        Current RN licensure, with no sanctions, restrictions, or provisions, in the state of Georgia.


·        2-5 years of related clinical, supervisory, or management experience in a clinical or home care environment


·        Demonstrated working knowledge of the Medicare and Medicaid programs, as well as the private insurance programs served


·        *Preference will be given to applicants with bi-lingual skills


 


Requirements:


·        Be able to work under minimal supervision.


·        Have the ability to manage multiple responsibilities simultaneously, often under pressure, and be flexible with regards to defining priorities.


·        Have excellent customer service skills.


·        Have the ability to handle sensitive and confidential information appropriately.


·        Possess attention to detail and strong organizational skills.


·        Be proficient with Microsoft Office applications.


·        Be able to effectively communicate both verbally and in writing.


·        Provide professional references.


·        Maintain documentation of a current negative TB screening.


·        Maintain documentation of current C.P.R. certification.


·        Submit to and receive satisfactory results of a criminal background check.


·        Have reliable transportation and the ability to travel as needed, and provide proof of current automobile insurance coverage.
