Job Details:
All the primary duties within this document will be performed according to established policies, procedures and guidelines within the department and the Medical Center.
JOB SUMMARY:
**Works under the supervision of the Director and Managers to prepare, label and dispense, as well as maintain appropriate records for all medications and other pharmaceutical supplies ordered by physicians or other qualified prescribers. Provides pharmaceutical care to all age groups: neonates (0-28 days), infant (1-12 months), child (1-12 years), adolescent (13-17 years), adult (18-72 years) and geriatric (72+ years). Works as part of a multi-disciplinary team to provide evidence-based, cost-effective care. Precepts pharmacy students. Work is accomplished in accordance with the policies and regulations of local, state and federal authorities as well as accepted standards of practice. Performs other duties as assigned.
PRIMARY JOB DUTIES:
Distributive Activities
1. Compounds, prepares, labels, and dispenses medications (sterile and non-sterile) for all age groups according to accepted standards of practice.
2. Performs computer order-entry functions in an accurate and timely manner.
3. Assists in maintaining an acceptable inventory of medications and pharmaceutical supplies under proper conditions of storage and security.
4. Supervises and verifies appropriate and accurate drug distribution tasks performed by support and technical staff.
5. Effectively uses all necessary forms of informatics.
Direct Patient Care Activities
1. Interprets medication orders, monitors drug regimens, and performs interventions when order clarification is necessary or when therapeutic problems are identified.
2. Meets and maintains competencies to perform the required duties of the position. See Department Requirement for Position Minimum Competencies for a Pharmacist II.
3. Assists with formulary and DUE program activities.
4. Promotes the use of cost-effective drug therapy.
5. Participates in the adverse drug reaction reporting process. Able to evaluate and complete ADR reports.
6. Documents activities per departmental policies.
7. Provides general drug information services and formally responds to general drug information requests.
8. Works with other health care providers, including participating in rounds and other patient care activities, to optimize medication regimens using evidence-based medicine and standards of care.
9. Provides services that maximize the hospital's ability to meet national indicators (CMS quality indicators, etc).
Educational Activities
1. Trains and orients new employees.
2. Assists in orienting others outside the department regarding the work of the pharmacy (i.e., physicians, including residents and students, nursing personnel, etc).
3. Counsels patients, families, and/or caregivers per departmental policies.
4. Participates in staff meetings.
5. Obtains and maintains Basic Life Support for Healthcare Providers (BLS) and Advanced Cardiac Life Support (ACLS) certification (if in acute care practice).
6. Serves as a primary preceptor for pharmacy students.
7. Provides educational information for professional and/or non-professional audiences on at least an annual basis.
8. Provides pharmacy staff education on at least an annual basis.
Administrative Activities
1. Assists with staffing problems as needed and is able to suggest ways to efficiently distribute work flow.
2. Follows accepted standards of practice and adheres to federal, state and local regulations, as well as the policies and procedures of the organization and department.
3. Supervises support personnel in a consistent and fair manner.
4. Demonstrates the ability to prioritize and effectively organize workload and problem-solve.
5. Communicates effectively in verbal and written interactions.
6. Participates in quality improvement projects.
7. Demonstrates the ability to implement new programs/improvements with minimal supervisory assistance.
8. Publishes in local publications, at least annually.
9. Participates in at least one departmental or hospital committee
ESSENTIAL JOB SPECIFICATIONS:
1. Education: Graduation from a School of Pharmacy accredited by the American Council on Pharmaceutical Education with a Baccalaureate or PharmD degree. Post-Graduate Year 1 Residency or equivalent.
2. Licensure / Certifications: Current registration and licensure by the State of North Carolina as a pharmacist.
3. Experience: Direct patient care required, pharmacy department will assess.
4. Essential Technical/Motor Skills: Hand-eye coordination; clear speech; knowledge of computer applications.
5. Interpersonal Skills: Good communicator; able to exercise good judgment and accept responsibility for completion of activities that may require extended hours and stressful situations.
6. Essential Physical Requirements: Able to lift 25 pounds, walk floors, climb stairs.
7. Essential Mental Abilities: Good judgment, analytical and critical thinking capacity. Work in stressful, demanding work environment 50% of the time. Must be able to maintain and control personal behavior to ensure positive demeanor at all times.
8. Essential Sensory Requirements: Able to use visual, olfactory and auditory senses.
9. Exposure to Hazards: Will be exposed to hazardous chemicals through the preparation of chemotherapy-Special equipment is provided for protection.
10. Other - Hours of Work: Primarily 8, 10, or 12 hour days, evenings or nights; but can be expected to work 8 to 12 hr shifts, rotating shifts, days evenings and nights including holidays/weekends, and multiple sites.
11. Population Served: Will provide care for all age groups from birth through geriatrics.
