JOB SUMMARY:
Provides pharmacy services (unit dose, I.V. admixture, basic clinical and outpatient
services) from the central pharmacy or assigned satellites. Consults with staff and
patients as requested. Assumes charge responsibility for the shift in the absence of the
Director, Assistant Managers and Senior Staff Pharmacist.
JOB SPECIFIC RESPONSIBILITIES:
1. Credits unused medications appropriately.
2. Maintains current Missouri Pharmacy License and Picture in department.
3. Participates in the department quality management program as requested.
4. Ensures appropriate disposition of medications returned to the pharmacy.
5. Fills initial orders as required.
6. Processes patient discharges and transfers to assure appropriate disposition of
their medications.
7. Ensures appropriate delivery and exchange of medication cassettes.
8. Reads and interprets all physician orders, clarifies questionable orders and
dosages, screens for potential drug allergies and interactions, and processes
approved orders.
9. Checks all work done by pharmacy technicians or interns for accuracy (e.g.,
prepackaging, order processing, unit dose packets and carts, IV admixtures,
compounding, labeling and narcotic dispositions).
10. Oversees filling the order and makes a final check for accuracy prior to
delivery to the nursing unit.
11. Coordinates activities with other staff members to facilitate efficient and
timely delivery and distribution of patient care services.
12. Conducts monthly nursing unit inspections or drug storage areas as assigned.
13. Conducts monthly controlled substances audits as assigned.
14. Monitors all IV fluid, IVPB and medication orders, checks for drug
incompatibilities and allergies, clarifies questions concerning the solutions
and medications requested, dosages and rates.
15. Delivers medications to and receives orders from nursing units.
16. Understands pharmacy and nursing practices in the hospital environment.
COMPUTER
17. Manages patient profiles as required.
18. Verifies technician orders in the computer system.
19. Uses additional MediTech modules including: PCI, Rescheduling, Resetting
Printers, Updating Allergies.
20. Enters new and standing orders in the computer system.
21. Generates canned labels as required.
22. Initiates reports from the system: Discharge List, Transfer List, IV
List/Labels, Cart fill list/catch-up list, IVPB Nightly Pull List and ASO List.
23. Retrieves and prints patient teaching monographs and Drugdex drug information
from the central pharmacy personal computer.
24. Enters floor stock charges into the system as required.
25. Inputs approved physician drug orders to the pharmacy patient profile, verifying
the number of doses needed for the day.
26. Debits and credits medications, duplicates labels and performs charge entry into
the computer system.
IV ADMIXTURE
27. Restocks the IV room as required.
28. Prepares IV admixtures, TPN's, Irrigation fluids and Chemotherapy accurately
using good aseptic techniques.
NARCOTICS
29. Dispenses narcotics to nursing units as required. Fills all controlled substance
prescriptions or requests according to policy.
30. Understands security requirements for controlled substances and narcotic lock
changing procedures.
31. Conducts narcotic audits, reconciliation and reporting.
32. Checks in controlled substances, handles/discards outdated, broken or used
narcotics per policy.
OUTPATIENT
33. Reconciles cash drawer.
34. Understands policies and procedures for prescriptions, documentation and filing.
35. Receives payment and prepares forms for prescription transactions including
charge cards, checks, cash and worker's compensation.
36. Advises outpatients on medications.
37. Dispenses over-the-counter medications.
38. Prepares employee prescriptions including filling, labeling, pricing and
labeling information.
EDUCATION
39. Completes an annual educational presentation for the department on a current healthcare topic.
CLINICAL
40. Member of Code Blue Team.
B. SECONDARY RESPONSIBILITIES
ADMINISTRATIVE
1. Assists with performance evaluation of pharmacy technicians and interns when
requested.
STOCK
2. Assists the buyer and Director in determining necessary stock levels.
3. Borrows and lends medications as required according to policy.
4. Prepares basic stock solutions.
5. Receives shipments to the pharmacy, processing associated paper work.
6. Uses and replaces floor stock per system.
MINIMUM REQUIREMENTS:
· Bachelor Degree in Pharmacy
· Other: Continuing education as required by the Missouri State Board of Pharmacy
SKILLS AND EXPERIENCE:
· 1 year in hospital pharmacy; may include internship or equivalent experience.
· Detailed knowledge of pharmaceuticals.
· Working knowledge of pathophysiology.
· Detailed knowledge of pharmacology.
· Detailed knowledge of drug preparation and packaging techniques.
· Basic knowledge of hospital operations and nursing practices.
· Knowledge of pharmaceutical law.
