Due to growth and expansion of clinical services, our client a state of the art, not for profit medical center with a progressive decentralized 24/7 pharmacy department seeks an Infectious Disease and Emergency Department Clinical Pharmacy Specialist.

Position Summary:

The Clinical Specialist Under the direction of the Pharmacy Director and Pharmacy Manager will implement new and existing pharmacy practice programs and initiatives for their specialty service line in a highly clinical de-centralized pharmacy model.  You will work on the floors in the, daily rounds, support and provide acute, patient focused, quality clinical pharmaceutical services, recommend cost effective drug therapy, collaborate with patients, peers and physicians, actively participate on committees and team development.

 

Responsibilities include but not limited to:

          Serves as clinical pharmacy specialist for Infectious Disease or Emergency Department specialty service line

          Participate in patient rounds; maintaining an expert clinical pharmacist presence at patient care area for which there is direct responsibility.

          Daily monitoring of patients to prevent, identify, and resolve drug-related problems, especially in highest-risk patients.

          Maintain monitoring Protocols: Kinetics dosing, Renal dosing, TPN orders, Microbiology review, IV to PO opportunity, and Open Intervention monitoring for this patient care area for which there is direct responsibility.

          Monitors patients for issues related to Core Measures and Patient Safety Initiatives.

          Daily communications with Physician and Nursing staff. Daily documentation via patient medical record and pharmacy clinical interventions.

          Educates patients/caregivers on drug therapy.

          Supervision of assigned pharmacist and support staff

          Design and implement hospital and departmental quality improvement measures for specialty service line.

          Participate at regional and national meetings whenever possible

          Be responsible for staff in-service, precept pharmacy residents and students

4 - 10 hour work schedule

Education/Experience Requirements:
Pharm D
PGY-1 Residency plus PGY-2 Specialty Residency preferred

Or PGY-1 Residency Plus 1-2 years Hospital specialty experience
Current State of Maine licensure or eligible
