CareMore's Primary Care & Collaboration division oversees strategy, operations, and care delivery in our primary care markets (Iowa, Tennessee, Connecticut), where CareMore builds and runs capitated primary care medical groups and currently serves over 22,000 high-risk Medicaid and Medicare patients, as well as our collaboration with Emory Health System, which brings the CareMore model to over 20,000 Medicare Advantage patients in Georgia. The division also supports the development of new markets and models in the Primary Care & Collaboration area, including multiple new markets planned in 2018 and 2019. Our comprehensive approach to care includes extensivists managing acute and post-acute episodes of care, primary care clinicians, behavioral health clinicians, care management & engagement with case managers and community health workers, and mobile home-based care. Clinical Pharmacist, CareMore Primary Care is a licensed pharmacist with a doctor of pharmacy degree and American Society of Health Systems pharmacist accredited residency and has any one of the listed board certifications. The pharmacist performs a variety of job functions in an ambulatory care clinic setting relating to the direct care of Medicaid patients.

Core Competencies

  • Ability to work collaboratively with an interdisciplinary team to provide pharmaceutical care to members by conducting Comprehensive Medication Reviews (CMR) for preferred drug list, polypharmacy, drug allergies, clinical effectiveness, cost effective drug selection, dose optimization, contraindications, adverse effects, potential drug interactions, therapeutic outcomes and adherence
  • Willingness to operate in an ambiguous environment, without continuous supervision
  • A collaborative team member, effective communicator, and coach for direct reports and other members on the team

Essential Duties and Responsibilities

  • Provide direct care to patients in clinic to initiate and prescribe new drug therapy, adjust, monitor or discontinue existing therapy under approved protocol for selected disease states including diabetes, CHF, COPD, hypertension, and hyperlipidemia
  • Review medication regimen with patient, in person or telephonically
  • Review pharmacy utilization reports to determine appropriate plan for pharmacy management, as applicable
  • Facilitate medication management to meet quality requirements
  • Participate in development of educational lectures and materials for CareMore clinical staff and patients
  • Partners with local in-network pharmacies for coordination in care and collaboration to meet patient needs.
  • Answer inquiries from physicians and nurse practitioners regarding clinical appropriateness, current guidelines and new drugs
  • Support pharmacy initiatives to improve quality outcomes and lower medication cost to members and organization
  • Understand and present reports on drug utilization and cost to providers and regional management
  • Serve as a member of the interdisciplinary care team

Work Location & Travel Expectations

Based in local market area.


  • 1 year or more of ambulatory care pharmacy experience including clinical practice (direct patient care) primary care experience AND American Society of Health Systems Pharmacists accredited residency
  • Board certified in BCACP, BCPS or BCGP required
  • Degree in Doctor of Pharmacy (PharmD.)
  • Board of Pharmacy Pharmacist License in required
