Physical Therapist Home Care

ACCEPTING DIRECT APPLICANTS ONLY - NO 3RD PARTY PLACEMENT APPLICANTS. Full-time and per diem positions. NEW PER DIEM RATES. The Staff Physical Therapist collaborates with other medical and ancillary personnel to develop treatment programs and rehabilitate patients. Specific components of the Home Care Therapy role include regularly re-evaluating the patient's therapy needs, initiating the appropriate preventive and rehabilitative procedures, assessing continuing care needs, preparing clinical and progress notes, coordinating services, informing the physician and other health care providers of changes in the patient's condition and needs, teaching and counseling the patient and family in meeting therapy related needs, participating in inservice programs supervising and teaching other personnel and coordinating and participating in the patients discharge planning.

* Maintains patient confidence and protects hospital operations by keeping information confidential.

* Maintains safe and clean working environment by complying with procedures, rules, and regulations.

* Protects patients and employees by adhering to infection-control policies and protocols.


* Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies.

* Develops physical therapy staff by providing information; developing and conducting in-service training programs.

* Complies with federal, state, and local legal and professional requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; advising management on needed actions.

* Contributes to team effort by accomplishing related results as needed.
