Position Title:                                                              Physical Therapist      

Department/Unit:        Chester River Home Care & Hospice (CRHC&H) is an affiliate of Chester River Health System now part of the University of Maryland Medical System. CRHC&H is located in historic Chestertown, Maryland. Chestertown is a waterfront, college town, surrounded by rolling farmland and ten miles from the beautiful Chesapeake Bay and 1 ½ hours from Baltimore, Washington D.C., Philadelphia and the beaches of the Atlantic Ocean.

Hours/Shift:                Full-time, Part-time or Per Diem

Flexible Days/Hours

Education:                               Licensed Physical Therapist in the State of Maryland and Graduate

                                                            from an approved APTA school.

Experience:                             Minimum one year of current therapy practice in a clinical setting.

                                                            Strong foundation in physical therapy clinical practice.

Other:                                                  Current CPR required.

Brief Outline of Essential Functions:

·         Under direction of the Practice Team Leader, administer physical therapy to patients requiring professional therapy services.

·         Act as a case manager for the patients receiving care; monitor insurance authorization.

·         Provide and instruct the patient and/or caregiver on physical therapy intervention as outlined the plan of care.

·         Conduct a comprehensive assessment of the patient on each visit and determine the appropriateness of home care.

·         Provide and instruct the patient and/or caregiver on physical therapy intervention

as outlined in the plan of care.

·         Document according to Chester River Home Care and Hospice policy and procedures for

admission and revisits which reflect skilled therapy intervention, the process of

assessment, identification and revision of goals, implementation and evaluation.

·         Communicate a plan of care and changes in the patient’s status to the physician, clinical

manager, insurance case manager and other disciplines as necessary.
