Commonwealth Medicine is seeking a Physical Therapist Consultant. The Physical Therapist Consultant is responsible for providing case consultation by reviewing medical records, conducting client assessments and making equipment and service recommendations. This position is responsible for recommending the authorization, coordination and facilitation of long term care equipment and services for program members.
As a member of a multidisciplinary team, the Physical Therapist Consultant is responsible assessing, planning, organizing, reviewing, and evaluating the care needs of members requiring community based services.
The ideal candidate will have the following qualifications:
- Bachelor’s Degree or appropriate degree required for Physical Therapy Licensure
- Appropriate licensure, registration or certification necessary to practice Physical Therapy in Massachusetts.
- 3-5 years work experience providing direct service to individuals with special needs and/or their families.
- Pediatric experience preferred
