Overview: The Physical Therapist (PT) is responsible for attending to patients in their homes according to the plan of care set up by the Physician and the health care team. In addition, the Physical Therapist is responsible for overseeing and advancing the physical therapy services delivered to our patients in their homes in accordance with a physician’s Plan of Treatment and state and federal program guidelines. This includes delivery of services to existing patients as well as identifying potential patients through marketing opportunities. Prospective candidates must have the ability to travel throughout the Metro Atlanta counties of Fulton, Cobb, DeKalb & Gwinnett and be licensed in the State of Georgia with no sanctions, restrictions, or provisions.

Qualifications for Physical Therapist:

§ Current Georgia License

§ Working knowledge of Medicare Home Health requirements

§ Strong assessment, case management and documentation skills

§ Experience working with Home Health and Oasis is preferred

§ Leadership and Case Management Skills

Salary: This is a position that pays $65 for a normal 45 minute visit and $75 for an admission visit with a mileage rate of 50.5 cents per mile. You can name your territory by choosing one through as many as you want zip codes within the four counties serves, Cobb, Fulton, DeKalb, Gwinnett.
