Physical Therapist - Outpatient Sports Medicine Attn: Outdoor and Sports Enthusiasts! Our client, a community hospital in the beautiful Pacific Northwest has recently hired a Sports Medicine Physician and is expanding their practice to include all sports medicine injuries. This rare outpatient sports medicine opportunity is located in Western Washington. The community is surrounded by trees, mountains, lakes, rivers and has quick access to Puget Sound and the Pacific Ocean. The compensation package includes a competitive base salary, comprehensive benefit package, sigh-on bonus and possible relocation assistance. Some PT experience is preferred, but new grads are welcome to apply. Job Description includes but not limited to the following: - Performs patient evaluations, assessing physical, social, and emotional needs; reviews patient medical record and physicians referral to determine treatment required
- Interviews patients, family members, and others to obtain relevant information which may affect the treatment plan-of-care
- Conducts tests and measurements of various organ systems (e.g. musculoskeletal, nervous, cardiovascular, etc.); selects and administers appropriate assessments (standardized and non-standardized)
- Assesses physical skills as they relate to educational, medical, social, and psychological factors
- Prepares written evaluation for patient's chart after initial evaluation, including a written plan-of-care as well as documentation of patient's stated goals
- Regularly re-assesses patient condition throughout treatment, modifying and changing treatment programs as indicated and based on individualized needs
- Plans and implements individualized patient treatment programs, defining short and long term goals which are both measurable and functional; monitors progress and treatment to modify therapeutic goals
- Selects treatment environments and activities according to individualized patient needs
- Provides counseling and guidance to patients and family. Acts as a resource for other services which might assist the patient and/or family
- Provides education, feedback and counseling to the patent and family about the patients physical condition and progress in treatment
- Follows all safety precautions set forth by P.T. and Rehab Department. Communicates immediately to department manager any safety incident or potential safety incident
- Works with the inpatient interdisciplinary team to provide care planning and discharge planning assistance
- Provides individualized written home and supplemental programs as needed. Instructs patient and/or family in techniques for safe transfers; practices safety precautions for all patients
- Adheres to all Medicare documentation provisions. Completes necessary paperwork to comply with standards and ensure proper coding and billing
- Maintains appropriate ethical standards
- Documents treatment provided and patient's response and progress in patient's chart
- Orients patient to the unit, explains purpose of activity, encourages patent activity, observes for signs of fatigue and adjusts activity appropriately
- Records patient changes on day of treatment and turns charges in to HUC staff
- Evaluate, recommends, fits and adjusts prosthetic and orthotic devices, wheelchairs, supportive devices, and other mobility aids
