- Provides professional nursing care for the safety and well-being of infusion home care patients.
- Collaborates with pharmacy staff to develop and monitor goals of patient therapy through the infusion pathway.
- Communicates with physician and other members of health care team as appropriate to convey information regarding patient status and to make recommendations.
- Coordinates patient referrals and admission with other members of the health care team in order to meet patient home infusion care needs and other care needs as assessed.
- Obtains correct, appropriate, and timely prescriptions and specific care instructions from referring physician, including prescriptions for all nursing orders.
- Demonstrates competency in venous access device management, infection control, and safe practice of infusion nursing.
- Identifies learning needs of patient and/or care giver. Provides appropriate intervention and instruction for patient/care giver to safely administer care at home.
- Communicates patient learning needs and findings to health care team members, including pharmacy, customer service staff, and physician/referral source, as appropriate.
- Appropriately documents patient care activity, home infusion nursing services provided and all communication with patient and physician.
- Documentation is accurate, complete, and complies with company policy and procedure for electronic and paper patient clinical record entries.
- Conducts ongoing chart review per policy.
- Forwards required documentation to physician, payors, and referral sources within policy or contractually stated time lines.
- On an on-going basis, monitors patient progress to collaborative home infusion goals to assess medical, psychosocial, and home care environment.
- Ensures patient competence and compliance with all self-care, including procedures and infection control; re-educates as necessary, and documents/communicates the need for re-education.
- Maintains current communication with physician regarding patient status and procedures.
- Provides nursing care and services in accordance with company policies and procedures, and all applicable JCAHO, federal, state, and local regulations.
- Troubleshoots problems regarding operational procedures. Responds to customer inquiries and resolves patient complaints.
- Participates in education and training conferences on patient care. Participates in training sessions of sales personnel marketing the nursing program.
- May assist sales personnel in medical aspects of marketing home care nursing programs.
- Regular and consistent attendance and punctuality is considered an essential job function of this position.
- Performs other related duties as directed by supervisor.
- Current licensure as a Registered Nurse in appropriate state of practice is required.
- At least one year related experience is required.
- Must possess a valid and current driver's license and auto insurance.
