The Commonwealth Fund, Qualis Health, and the MacColl Institute for Healthcare Innovation at the Group Health Research Institute have launched an initiative to help primary care safety net clinics become high-performing patient-centered medical homes. The goal of the Safety Net Medical Home Initiative is to develop a replicable and sustainable implementation model for medical home transformation. What you’ll do:
You’ll coordinate the planning, delivery, and tracking of technical assistance delivered to Regional Coordinating Centers and their Partner Clinic Sites under the Safety Net Medical Home Initiative; also lead the Community of Practice and play a key role in workgroups, particularly the Technical Assistance Advisory Workgroup.
What you bring:
You have at least three years of related work experience in a healthcare setting that includes at least two years of experience with quality improvement, project management, and/or external consulting plus an Associate’s or Bachelor’s degree in a related field. Master’s prepared, CPHQ, or CPM preferred.
