| Assessment/Reassessment Interviews patient and other individuals to collect information. Observes and analyzes physical condition, including performing physical examination of the patient. Reviews medical records, patient history, and other documentation. Reassesses patients as necessary when a patient is currently or potentially at physiologic, psychological, or psychosocial risk. Recognizes deviations from baseline and interprets subtle changes in patient's condition. Planning Formulates nursing care plan based upon assessment and in accordance with provider orders, procedures, and/or protocols. | This examination will consist of two (2) parts: PART I: A qualifying written test covering common abbreviations and medical calculations. A HAND HELD BATTERY/SOLAR OPERATED NON-TAPE PRODUCING CALCULATOR WILL BE PROVIDED AT THE WRITTEN TEST. USE OF MEMORY-STORAGE, SCIENTIFIC, STATISTICAL, OR FINANCIAL CALCULATORS, CELL PHONES, PERSONAL DIGITAL ASSISTANTS (PDAs), AND ELECTRONIC DEVICES IS PROHIBITED. ONLY CANDIDATES WHO ACHIEVE A PASSING SCORE OF 70% OR HIGHER ON THE WRITTEN TEST WILL PROCEED TO PART II. THE WRITTEN TEST IS NOT REVIEWABLE BY CANDIDATES PER CIVIL SERVICE RULE 7.19.
