JOB SUMMARY
The Registered Nurse manages all aspects of patient care and provides skilled nursing care to patients in their home. This includes direct patient care, supervising health aides, referring to community resources and/or utilizing resource materials as needed. Includes participation in case conferences and staff meetings; completing paperwork in a timely manner, conferring and meeting with physicians, discharge planners, and other community persons as needed or directed. Monitors quality of service and discipline, providing information on patient’s condition to physician and supervisor. Resolves patient/family concerns.
ESSENTIAL JOB FUNCTIONS
Comprehensive Nursing Care
A. Demonstrates appropriate physical assessment skills for initial and follow-up appraisal of patient status and possesses an adequate knowledge of clinical disease entities.
B. Demonstrates ability to perform nursing treatments and procedures as prescribed for home health patients.
C. Demonstrates adequate knowledge of specialized diets and medications commonly prescribed to home health patients.
D. Demonstrates knowledge and awareness of rehabilitative and preventive aspects of care and ensures patient safety, comfort, and protection.
E. Teaches patient/family care needs and health information at their level of understanding to facilitate independence and/or recovery.
F. Demonstrates wide knowledge and appropriate utilization of community resources and support of related Eskaton and Community Services.
Documentation
A. Documents notes on each patient reflecting skilled care, patient instruction, and patient/family response, and specific plan for ongoing care needs.
B. Submits timesheets and written plan of treatment reflecting patient status and nursing care plans in a timely manner.
C. Submits required documentation which consistently meets home health and reimbursement regulations and guidelines within the time frames established by the agency.
D. Documents using the electronic documentation system.
Case Management
A. Coordinates service among all disciplines including outside agencies.
B. Communicates information regarding the patient at team meetings and coordinates care provided to the patient.
C. Confers with others as needed to plan and implement a realistic/reasonable plan of care and confers with home care coordinators or discharge planners regarding hospitalization of patients and agency involvement.
D. Establishes outcome based on Plan of Care with short term and long term goals for total patient care and management, ongoing assessment of outcome achievement.
E. Supervises home health personnel providing patient care per state and federal regulations and agency guidelines.
QUALIFICATIONS
Education:
Graduation from a Baccalaureate Degree Nursing Program or Diploma School of Nursing or an Associate Degree Nursing Program. Possession of a valid California Registered Nursing license. Current CPR Certification.
Training and Experience:
One year recent home health nursing preferred. One year in acute care setting.
Able to learn to operate a laptop computer to perform clinical documentation and keyboarding or typing skills; previous experience preferred.
Knowledge and Abilities
Possesses an in-depth knowledge of comprehensive nursing care which includes: clinical disease entities common to assigned home care patients; the physical assessment skills required for initial and follow-up appraisal on all assigned patients; the treatments, specialized diets, and medication regime for individual cases opened to services; the rehabilitative and preventative aspects concerning each patient; additional direct patient care service which ensures the safety, comfort, and protection of patients; the usual performances indicated for disease prevention and restorative measures; ability to document professional nursing services provided with a realistic plan for reaching short term and long range goals; able to make decisions regarding safety needs of patients' home including adequate equipment.

Possession of a valid California driver's license, access to an operating automobile, and at least minimum coverage automobile insurance.
