We are seeking a Home Health RN Case Manager for our New Medicare Agency.

This important position includes the planning, development, and implementation of individualized resident care plans designed to assist the resident in reaching the highest practicable level of physical, mental and psychosocial well-being.

As a registered nurse, you are delegated the administrative authority, responsibility and accountability necessary for carrying out your assigned duties. You report directly to the Director of Nursing or charge nurse or shift supervisor.

Primary Responsibilities:

•Plan, develop, and implement individualized resident care plans as applicable, and in accordance with current federal, state and local standards, guidelines and established policies.
•Efficiently plan, schedule, supervise, perform, document information for treatments provided and services rendered, in accordance with standards of care and established policies and procedures.
•Maintain an excellent working relationship with other departments and coordinate the care provided to assure that resident needs are met in an interdisciplinary manner.

- Travel is minimal as all clients are on one Campus.

- On-call requirement is once/month.

Minimum Requirements:

•Current state RN licensure and at least 1 year of recent MEDICARE HOME HEALTH experience.
•Must exhibit skills in performing procedures, assessments and tasks included in the scope of practice, effective communication, policies, education, community relations, and nursing services as they relate to resident care.

- Must be computer literate.
•Must possess the ability to deal tactfully with co-workers, personnel, residents, family members, government agencies and the general public.
•Must be willing to maintain safe work habits and in accordance with safety policies of the facility at all times.
