Magnolia Management, Inc is an operator of skilled nursing facilities located in Maryland and Pennsylvania. All of our facilities offer long term care and short term rehab care. Magnolia Management, Inc is currently seeking an RN-Clinical Case Manager to join our dedicated team! The position is responsible for facilitating the admissions process for the 5 facilities which are managed by Magnolia Management, Inc. The Case Manager will be responsible for updating the local hospitals, pertinent case workers, and other relevant sources within the surrounding communities regarding the bed availability at each of the buildings. This position will be at our corporate office in Hagerstown, Maryland and will supervise two employees but will also be remotely working closely with Admissions Coordinators in each of the facilities. This position will report directly to the corporate Chief Operating Officer.

The essential responsibilities of the position include the following:

Evaluates referrals to determine admission candidacy.

Evaluate the client’s insurance payer sources.

Keep an open line of communication with all referral sources.                                  
Notifies hospitals of bed availability on a daily basis.

Communicates with the facilities Admissions Coordinators regarding admissions and discharges within the facilities.

Supervise and directs all aspects of the central admissions department

Produces reports as requested.
Must have billing experience also

The position is full time, Monday-Friday, with some overtime work. Excellent benefits package!

Qualifications:

Requires Bachelor's degree in a related field. Masters preferred.

Current RN license in the state of Maryland.

Experience in which supervisory abilities have been demonstrated.

High level of accuracy and dependability in decision making and an excellent ability to evaluate situations as they occur.

Proficient in computer based programs.
