Position Description:

Our new Portland, OR office is seeking seasoned RN's to provide telephonic disease management services to our member population. The hours for this position are Monday - Friday 10:00am - 7:00pm.
Health Coaches are responsible for providing disease management, training, and other clinically based activities as assigned to coordinate care between providers, the clients and the community. They will perform telephonic disease management assessments to empower clients to better understand their illness and self-manage their condition. Education is specific to disease processes, nutrition, wellness, etc. The Health Coach will make adequate contact to clients, as stated in Program guidelines with a focus on behavioral goal setting and client centered support planning. Additionally the Health Coach will identify member resource needs and provide information on possible resources to clients and make referrals for clients, when applicable.
Responsibilities: • Conducting on-site healthcare facility visits to facilitate quality of care. • Assess, plan, implement, coordinate, monitor and evaluate the treatment plan and/or develop alternative options. Establish short and long-term goals in collaboration with the client, the client’s health care team and the plan. • Coordinate services, and assess progress and outcomes. • Provide education on the disease process. • Meet performance standards. • Attend and participate in case conferences. • Other duties as assigned.
EDUCATION: Registered Nurse with active non-restricted license for the State of Oregon.
REQURIEMENTS: • Minimum of 2 to 3 years experience in related public sector medical services including experience with quality improvement, medical record reviews, disease management and/or primary care case management. • Knowledge of chronic illness and disease states targeted by DCH. • Experience and/or knowledge of telephonic based communication. • Strong verbal and written communication skills. • Above average computer/technical skills; • Ability to resolve problems in a non-threatening manner.
