Home Health & Hospice Visit Nurse Job Description
PRI's current client is a top Medicare-Certified Hospice and Home Health agency (Parts A & B) entailing all skilled services, all insurances, Medicaid, private duty skilled care, home health and hospice. They are currently looking for a dual role Home Health & Hospice Visit Nurse.
This position will start work from the home and visit patients in their home. It is expected that the qualified RN will come into the office each day to collect and/or drop off paperwork. The majority of the work will be within Home Health (census of 30) but there is a small Hospice patient census as well to which this nurse will be accountable. Having functional knowledge of Home Health OASIS paperwork is very important. The geography for this position will be from South Bountiful up to North Ogden. However, the majority of the work will actually be done within a 15 mile radius comprising Clearfield, Layton, Roy and other nearby areas.
The qualified candidate must have very good communication skills, be systems oriented and innovative in their thinking. A Home Healthcare or Hospice background would be beneficial. Knowledge of Medicare/Medicaid, compliance functions and regulations therein would also be helpful.
Some of the requirements of this position will include:
· On-time completion of all necessary paperwork for billing purposes
· IDT meeting attendance one day per week
· Conduct initial admissions and scheduling for future physical therapist visits.
· Basic overall home health care and hospice coordination
· Home health and hospice medical records oversight
· Basic on-call
· Occasional client visits for quality assurance, nursing supervision, etc.
· Helping the Wound Care RN and visa versa, as needed
The qualified candidate must have a current, valid RN license from State of Utah with the ability to practice with no restrictions. This person must have clear leadership and team building skills. A compassionate attitude is extremely important with a great desire to make a difference in patient’s/caregiver’s and company’s lives. This is a full-time salaried position with additional competitive health care benefits, PTO, car allowance, flex time, holidays, some variable performance bonus, etc. Salary is negotiable depending upon experience. The qualified person will be very knowledgeable and comfortable with basic computer software and PC functioning.
