Bayada Nurses, Home Care Specialists, is seeking a full-time Registered Nurse to fill the position of Hospice Clinical Manager in our Norwich, Vermont office. The successful candidate must be a Registered Nurse who has demonstrated clinical and managerial experience within a Medicare Certified Hospice Agency. As Registered Nurse Clinical Manager you will have responsibility to direct and coordinate the care provided by an interdisciplinary team of home health care professionals. Job Responsibilities include: processing incoming referrals, management and mentoring of field staff, interdisciplinary group planning, reviewing all clinical documentation, monitoring quality assurance, and overall case management. Requirements: Current VT and NH Registered Nurse licenses. 3 years nursing experience. 2 years recent Medicare Certified Hospice experience. In-depth knowledge of the new Hospice Medicare Conditions of Participation. Strong management and leadership background. Excellent organizational, interpersonal and communication skills.
