As a RN Manager – Manager of Clinical Practice, you will:
•Conduct or delegate the assessment and reassessment if patients, including updating plans of care and determining patient needs, while adhering to Company, physician, and/or health facility procedures or policies. Manage the assignment of caregivers.
•Coordinate communication between team members, attending physicians, caregivers, and community resources to ensure the appropriateness of care and outcome planning and ensure compliance with third party requirements through review of documentation and care coordination activities.
•Manage and maintain clinical assessment, database, and utilization information while ensuring safe patient care.
•Ensure compliance with standards, company policies/procedures, and external regulatory requirements. Ensure that caregiver clinical documentation meets these requirements.
•Consult with third party representatives regarding coverage, patient financial responsibility, service authorization, and reimbursement procedures. Negotiate reimbursement levels and/or facilitate this process.

Qualifications
•Associate’s Degree or Diploma in Nursing or the equivalent
•Minimum of three years nursing experience in area of specialty preferably in a community or homecare setting
•One year in a supervisory role generally required
•RN licensure in the state(s) of desired practice
•Excellent organization, problem-solving, ad communications skills
