RN-REGISTERED NURSE-HOSPICE (FT ON-CALL Weekdays)
FULL-TIME ON-CALL RN(Weeknights from 4pm-8am)
This position will cover all of our territories in Somerset, Morris, Warren, Hunterdon, Sussex, Passaic and Union counties.
YOU ARE PAID IF YOU GET CALLED OR NOT!!!!
- Case management of hospice patients in long term care and home settings.
- Management of nurses aides for your team.
- Development of individualized plans of care for patients within 48 hours of admission.
- Provision of direct care, education and support to patients and family members.
- Management of medical regime as prescribed by attending physicians.
Requirements
- Current and valid RN License
- 1 year administrative or supervisory experience
- experience in either Hospice, home health, primary care clinic or other health facility
- Strong communication skills
- Reliable transportation
