RN-Registered Nurse -Hospice Case Manager

    This position covers the Riverside area!

  • Case management of hospice patients in long term care and home settings.
  • Management of nurses aides for your team.
  • Development of individualized plans of care for patients within 48 hours of admission.
  • Provision of direct care, education and support to patients and family members.
  • Management of medical regime as prescribed by attending physicians.

    • Requirements
    • Current and valid RN License.
    • 1 year administrative or supervisory experience.
    • experience in either Hospice, home health, primary care clinic or other health facility.
    • Strong communication skills.
    • Reliable transportation.
