FT RN-Hospice Case Manager

This position covers towns in Riverside County!

  • Case management of hospice patients in long term care and home settings.
  • Management of nurses aides for your team.
  • Development of individualized plans of care for patients within 48 hours of admission.
  • Provision of direct care, education and support to patients and family members.
  • Management of medical regime as prescribed by attending physicians.

    Requirements
    • Current and valid CA RN License
    • Experience in either hospice, home health, primary care clinic or other health facility
    • Strong communication skills
    • Reliable transportation.
