JOB QUALIFICATIONS
Education, Training, and Experience; Certification/License
The RN Supervisor must meet one of the three educational work experiences as outlined below
1. A baccalaureate degree in nursing and at least two (2) years of full-time clinical experience within the past five (5) years in a home health care agency or related health care facility program which included care of the sick; or
License/Certification if Applicable:
Licensed as a Registered Professional Nurse in Connecticut
Certification in public health or home health nursing preferred

Exposure to patients-

Administrator, Case Coordinator, Patients, Family, State agencies, home health agencies, community organizations and other members of a patient’s interdisciplinary treatment team

A--JOB DESCRIPTION----Summary of Job Purpose
The registered nurse supervisor shall have primary authority and responsibility for maintaining the quality of homemaker-home health aide services provided to the patient.
B- JOB RESPONSIBILITIES
Job responsibilities are those that cannot be easily transferred to another job, and if removed, would significantly change the nature of the job.

Completes the initial assessment of the patient and the home situation so as to determine that the patient’s status and care needs can be safely met by Task Medical Home Health Aide Service

2 | Refers the patient (at time of admission and at any time during the service period) to a home health care agency or other appropriate level of care, when the patient’s status and care needs requires more than the supportive services as defined in 19-13-D80 (z) of the Connecticut Department of Public Health Code.

3 | Develops a written plan of care for the patient which shall include the frequency of assessment and methods by which the patient’s status and care needs are to be monitored between assessment visits in the home.

4 | Includes the patient and family/significant others in the development of the patient’s plan of care. Serves as the “coordinator’ for the services rendered to the patient and family. |
5 | The plan of care shall be reviewed and revised as often as necessary based on the patient’s condition, but not less frequently than every sixty (60) days. |
6 | When appropriate, communicates with the patient’s source(s) of medical care to secure or report information pertinent to the patient’s care. |
7 | Development of written instructions for the homemaker-home health aide, which shall be completed before the homemaker-home health aide provides any service to the patient. o The instructions will be reviewed and revised if the patient’s condition changes but not less frequently than every sixty (60) days.

8 | The The written instructions for the homemaker-home health aide shall include: o scope and limitations of homemaker activities o pertinent aspects of the patient’s condition to be observed and reported to the registered nurse supervisor o the name and telephone number of the registered nurse supervisor.

9 | Participates in the interviewing and selection process for the recruitment of homemaker-home health aides to provide services for Task Medical Staffing.

10 | Pr Provides staffing input and support as necessary to the case coordinator to ensure patient staffing assignments. |
11 | Dev Develops and implements an orientation plan for the homemaker-home health aide by: o providing: an expanded orientation program in a class-room setting o a “summarized verbal and written orientation when patient assignment will take place prior to the scheduled “expanded” orientation. o an introduction of the HHA to the patient and family on the first visit.

12 | Evaluates the competency of the homemaker-home health aide to carry out all assigned patient care activities o through direct observation in the patient’s home. o a review of the records and reports prepared by the homemaker-home health aide o the effectiveness of the homemaker-home health aide participation in the patient care conferences

13 | Completes the necessary performance evaluations on all orientees as outlined in the orientation policy and procedure. |
14 | Implements on-going in-service programs based on both state required annual topics and findings from the quality assurance monitoring process. |
15 | Completes the annual performance evaluation on all homemaker-home health aides’ |
16 | Collaborates with the administrator to ensure that there is a process to receive feed back on employee performance so as: o to recognize satisfactory performance o to coach when necessary to improve performance o to implement the disciplinary process when

Participates and or coordinates the investigation of patient complaints as assigned by the administrator within the time frames and documentation requirements of the Complaint Policy and Procedure.

18 | Provides coaching and additional training as necessary to improve a homemaker –home health aide’s performance. Implements the disciplinary process when coaching and training have been ineffective and or when a homemaker-home health aide flagrantly violates established policies and procedures of Task Medical Staffing INC.

19 | Develops the format and process for maintenance of the patient care record. |
20 | Participates in the development of a quality assurance monitoring program in collaboration with the administrator and other members of the operations team and or governing board.

21 | Implements components of the quality assurance monitoring program as assigned by the administrator |
22 | When appropriate, the RN supervisor may delegate all or part of the professional supervision of a homemaker-home health aide to physical therapist, occupational therapist, speech therapist or social worker as appropriate. In such situations, the RN supervisor shall review the patient’s plan of care at least every four ( 4) weeks with the designated supervisor

23 | Participates and collaborates with the administrator and the Director for Clinical Services in the development of the operating and patient care related policies and procedures which will also include plans for medical or other emergencies

24 | Develops the quality control plan and assigns implementation to the operations team that is outlined in the detailed Quality Control Plan |
25 | Provides input as necessary to ensure that the established client and employee policy and procedures meet regulatory compliance and service excellence

26 | Finds ways to improve work processes and other procedures to maximize resources and reduce costs without impairing the quality of client services. Reviews trending data on organizations’ complaint, incidents, satisfaction surveys as tools to improved existing workflow

27 | Promotes staff satisfaction and teamwork through effective communications Communicates all information in a clear and timely fashion |
28 | Supports, follows and administers all policies and procedures as required. Example: Follows all time and attendance policies, completes reports on time.

Functions in a problem solving mode when necessary displaying analytical skills, critical Thinking, innovation, creativity, judgment and an ability to anticipate outcomes.

30 | .Ensures that work areas and equipment in department (or area of responsibility) are maintained in safe condition, according to standards and specifications

32 | May serve as the administrator for vacation relief when there are ten (10) or less full time homemaker-home health aides. |
