Chartwell Midwest Wisconsin is seeking a Registered Nurse to work either at our office in Middleton, WI, or at our office at the University of Wisconsin Hospitals and Clinics site in Madison, WI.  Our nurses coordinate and facilitiate services for patients receiving home infusion.  This includes both the admission and referral process as well as ongoing interaction with the patients, staff, physicians, referral sources, and community agencies. 

Chartwell has been servicing patients since 1994 and we pride ourselves on great customer service and patient service, as well as high quality clinical management.  We are Joint Commission accredited.

Candidates best suited for our work environment will have 2 years experience in acute home care, home health, or infusion nursing.  Successful candidates will also possess excellent organizational, interpersonal, and customer service skills. 

Requirements include:  State licensure as a Registered Nurse, pre-employment drug screening, and criminal background check.
