Description
Identifies patient/family/caregiver needs and provides nursing and supportive care in accordance with the patient’s plan of care, and VistaCare’s policies, procedures and care processes.

Principal Accountabilities
1. Applies nursing principles and techniques in the treatment of patients. Provides nursing care, emotional support and education to the patient/family/caregiver.
2. Responsible for the development, implementation and continued development of the patient care plan and physician’s plan of treatment.
3. Responsible for the case management of the patient, including supervision of all nursing and Certified Nursing Assistants.
4. Checks and logs temperature of medication refrigerator daily.
5. Communicates with physicians and others as needed to coordinate and ensure high quality care.
6. Communicates on a regular basis with all staff/volunteers regarding patient status.
7. Attends and participates in shift report, team meetings, briefings, in services, department and staff meetings and other related activities.
8. Understands and adheres to the VistaCare Mission, Policies and Procedures and advocates its philosophy.
9. Works effectively in an interdisciplinary team environment.
10. Follows the guidelines for universal precautions when providing patient care.
11. Demonstrates a positive attitude and working relationship with staff members and others.
12. Maintains records of service performed and of apparent condition of patient, using documentation guidelines and standards set forth by VistaCare.
13. Carries out duties and responsibilities of position in a manner that exemplifies excellent customer service.
14. Consistently represents VistaCare to all internal and external customers with integrity and professionalism
15. Answers telephone, identifies department and self, directs call to appropriate person or assists caller. Takes messages, documenting the time, name of caller, phone number and message, when applicable.
16. Addresses patient’s needs by being responsive to call lights/bells.
17. Maintains strict confidentiality of patient/family/caregiver information.
18. Provides for a safe environment for patients, guests, visitors and employees.
19. Adheres to all policies, procedures and regulations regarding patient care,
conduct, safety, fire, security, and risk management.
20. Adheres to VistaCare Standards of Conduct.
21. Has a duty to report any potential misconduct or violation of VistaCare Standards of Conduct or federal or state laws and regulations.
22. Has a responsibility to participate in staff training and education programs.
23. Performs other duties as assigned.



 


Requirements


Qualifications
1. One year of recent medical, surgical, oncology or home care experience.
2. Registered Nurse with current state license.
3. Recent acute care experience a must.
4. Complies with acceptable professional standards and practice.
5. Knowledge of pain and symptom control related to terminally ill patients.
6. Demonstrates good communication, negotiation and public relations skills.
7. Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
8. Reliable transportation, current driver’s license and current auto insurance.
