CHS, a leading occupational health provider is seeking a RN-Health Coach in Guymon, OK. Work in our onsite health unit clinic at Seaboard Foods to help individuals achieve their personal health goals. The coach typically provides services while onsite at the workplace of our corporate clients. The health coach conducts onsite health risk assessments, provides wellness education, supports individual lifestyle risk reductions, helps individuals with chronic diseases manage their conditions, and provides onsite minor acute care and is a case manager for individuals with complex and demanding situations. Additionally, the onsite coach provides triage and consultation services to health related inquiries from the corporate clients.
Qualifications:
Must be licensed as a registered nurse in the state(s) within which they work, with a minimum of 3 years professional, clinical experience. Knowledge and experience in health risk assessments, lifestyle risk reduction, disease management, case management, and health promotion is recommended. Prefer bilingual in Spanish.
