position description

Registered Nurse/Home Care

Perform nursing care in patient's home to administer treatments, complete all appropriate documentation. Supervise Home Health Aides, carry a bbeper for on-call rotation, teach patient and caregiver, complete/modify care plan/CMI, track and document patient's progress toward stated goals, adhere to state, federal and hospital regulations, policies and procedures. Demonstrated proficient management of complex infusion therapy patients including the use of a variety of central lines.
