Care Coordinator / Case Manager
As a care coordinator / case manager at Baylor, you’ll know you’re making a difference in your patients’ lives. As a lead patient advocate, you’re the extra voice ensuring they get the quality care they need when they need it.
Baylor’s nurses are empowered to make decisions that improve patient care. We are committed to a shared leadership model that encourages all staff nurses to be actively involved in setting the clinical policies and procedures for their unit.
We use the title care coordinator instead of case manager, because their role is caring for patients rather than managing cases. Care coordinators do far more than communicate with insurance companies. They improve patient outcomes by overseeing the continuum of patient care and providing a vital link between patients, payers and staff. Care coordinators use all hospital and community resources to ensure that patients are on the road to recovery after they leave the hospital.
Care coordinators have many resources available to them, as several departments at Baylor help support care coordination throughout the system. Our Managed Care department works closely with care coordinators to ensure they have a voice when there’s an issue with a payer. Our Denial Resource Center also steps in to assist when needed. Through our Inpatient Document Integrity program, our care coordinators work with physicians and Health Information Management (HIM) to ensure charts are coded correctly.
In a spirit of teamwork, there’s a strong collaborative relationship between care coordination and other departments, which helps create a positive work environment for care coordinators. Care coordination and social work enjoy an especially productive relationship at Baylor.
We have career opportunities available for care coordinators on all shifts, including days, nights and PRN, at all of our system hospitals and Two-Day Alternative schedules at some hospitals.

Requirements
If you are an RN with an associate degree or preferably a bachelor’s degree, at least one year of nursing experience and case management experience, consider joining Baylor.
