Registered Nurse / RN / Home Health
Full Time & Per Diem
RESPONSIBILITY:
- Provides professional nursing care to the patients/clients in the home care setting
QUALIFICATIONS: - Registered Nurse, Licensed in the State of Florida
EXPERIENCE: - One year nursing experience, preferred;
- Specialty Nurses must have demonstrated knowledge and competency in their field
JOB KNOWLEDGE:
- Knowledge of home health nursing techniques, including: OASIS, assessment; Care Planning and others; Skilled with interpersonal communications.
OTHER:
- Skilled with clinical documentation.
- Maintains current CPR certification.
- Ability to work with limited supervision.
- Good organizational skills.
- Demonstrated competency for skills required by specialty.
- Ability to supervise home health aides.
- Computer user skills preferred.
FUNCTIONS:
- Conducts comprehensive home health patient assessment
- Performs nursing procedures, including but not limited to: administration of medications, catheterization, wound care/sterile and non-sterile dressings, decubitus care, enemas and ostomy care, and others.
- Teaches patients, family members and other caregivers.
- Orients and supervises para-professionals to the home health care plan.
- Communicates nursing assessments and diagnosis to Clinical Supervisor and/or Primary Care Physician as appropriate.
- Documents nursing assessments and observations accurately, objectively and concisely
- Initiates appropriate preventative, therapeutic and rehabilitative nursing procedures and techniques with consistency to physician orders for care
- Participates with care coordination with the physician and others of a multi-disciplinary team toward positive outcomes.
- Implements a plan of care to achieve positive outcomes.
- Responsible for discharge planning and referrals for continued community or other supports.
