Registered Nurse (RN) Intake

About the Job


Senior Health Partners is seeking an experienced, self motivated, organized and outgoing Registered Nurse to assist the Enrollment Department in conducting initial clinical assessments to determine member eligibility in our Managed Long Term Care Plan. Excellent team environment and supportive staff.  Full time, part time and per diem positions available. Field transportation coordinated. Office sites in Manhattan and Brooklyn. Bilingual Spanish, Chinese speaking preferred and home care experience desired. Flexible hours are available.     


 


Responsibilities:  


1.  Conduct enrollment assessments in the homes to determine clinical eligibility for enrollment using the State approved SAAM, 485 and other internal documents.  


2. Communicate and coordinate with the SHP Welcome Team services to be put in place for new members  


3. Communicate with referral sources, community agencies, physicians and family members regarding service plans and bridging care between programs.  


4. Completes all required eligibility forms and summarizes clinical needs, service plan into a detailed narrative for submission to Human Resource Administration.  


5. Work closely with the marketing and provider relations staff within SHP to promote health and awareness of SHP services to optimize referral business and build professional relationships.  


 


Requirements:  


1. NYS Registered Nurse with Home Care, Managed Long Term Care or Nursing Home experience


2. Ability to be independent critical thinker, self-directing and flexible


3. Warm, friendly, outgoing personality with strong interpersonal skills


4.  Excellent communication skills, along with ability to lead small and large group discussions as group leader


5. Ability to summarize in written form assessment findings and plans of care


6. Knowledge of computers, data entry, including Word, PDF. Training/computer provided.


7. Desire to work with geriatric population
