Job Summary: Total Homecare Solutions is a provider of Supported Living Services for individuals with Mental Retardation and Developmental Disabilities. We are currently looking for a Registered Nurse to oversee and manage all medical and clinical aspects for the individuals we serve. The RN position will coordinate and supervise Direct Care Staff and HHA’s in the delivery of patient care and maintain compliance with agency policy and procedures, as well as Medicare and Medicaid regulations.
Qualifications/Educational Requirements:
1. Be a registered nurse (R.N.) with a current license.
2. Minimum of eighteen months of nursing experience as a R.N.
3. Work positively and favorably with patients, families, Doctors and staff.
4. General computer knowledge preferred.

Responsibilities/essential functions: The person in this position must be able to perform the following essential job functions with or without reasonable accommodations.
1. Perform the initial home care patient visit and re-evaluate the patient’s needs and progress on a regular basis. *
2. Initiate the plan of care under doctor’s orders. *
3. Perform admission, transfer, re-certification, resumption of care, and discharge for the patient. *
5. Monitor reactions and patient progress. *
6. Educate patients and caregivers on disease process, medications, plan of care, and individualized treatment plans. *
7. Educate patients and caregiver on techniques for in-home health care. *
8. Coordinate patient services including medication management and all medical appointments. *
9. Supervise all medical care and reporting of Direct Care Staff and HHA’s. *
10. Notify the physician and other personnel (DON, PT, Case Manager, and Guardian) of changes in the patient’s condition. *
11. Update knowledge and skills by attending in-service programs, continuing education programs, seminars, and self-study programs annually. *
12. Provide onsite supervision of Direct Care Staff. *
13. Train and delegate Direct Care Staff to administer medications in accordance to County and State policy.
14. Maintain all clinical records in accordance to County and State policy and procedures.
15. Provide quarterly assessment of staff skills in administration of medication and delegate license to those staff that meet standards.
16. Conduct in-service training for staff as needed and present medical/educational training to staff at least monthly.
17. Work with main office to assist in the development, planning, and implementation of protocols, policies, procedures, forms and reporting of health and wellness related issues of MRDD programs.
