Job Summary
Responsibility for providing home care services consistent with the mission and goals of the Agency. The RN is responsible for providing skilled nursing services under a plan of care developed by the Clinical Supervisor and the patient's physician. The RN provides the skilled nursing services necessary to adhere to the plan of care. They include, but are not limited to, patient assessment, evaluation and coordination of the medical nursing needs of the patient; the performance of prescribed medical treatment; the provision of emotional support to both the patient and his/her family; and the instruction of caregivers in providing care to the patient. The RN shall provide for the continuity of services for the patient and his/her family. The RN is a professional member of the home care team.
EDUCATION, EXPERIENCE, AND TRAINING
Must have (1) year minimum of Home Healthcare Experience!
§ Licensed by the Arizona Board of Nursing, pursuant to ARS Title 32, Chapter 15 including fingerprint checks.
§ Current CPR and TB clearance.
§ Successful passage (80%) of Agency competency exam, clinical skills self-assessment and core competency per skills checklist.
§ Minimum of one year of HOME HEALTH experience.
§ Complete 6 hours of in-service training per year.
