This $100 Million dollar project expected to open in 2011 will include the city’s first Urgent Care facility, as well as the Elliot 1-Day Surgery Center, diagnostic imaging, breast health, endoscopy, pain management, physical rehabilitation, pulmonary rehabilitation, laboratory services, durable medical equipment, and more, including an 800-car parking garage.

Department and Position Overview:
If you’re seeking to bring your social work expertise to a progressive health system and to an exciting and dynamic Geropsychiatric Unit, then this opportunity and our future departmental initiatives will appeal to you. Elliot’s Geropsychiatric Unit is seeking to create a family caregiver educational group, and to develop a Montessori-based intervention program for our behavioral patients. The 24-bed Geropsychiatric unit, with two Social Workers and an average daily census of 22, is part of a continuum of geropsychiatric care. Equally as positive is the department’s use of an efficient Electronic Medical Records (EMR) system. We are an interactive unit, a very supportive and supported environment, and a unit for which people truly enjoy working!
Reporting to the Director of Behavioral Health Services, a very supportive and open-minded individual, who encourages autonomy, the Social Worker will conduct initial assessments of patients and of patients’ families and foster positive relationships with both parties, as well as with all internal and external staff. The incumbent will collaborate with an interdisciplinary team including Psychiatry, Nursing, Social Work, and Occupational Therapy to create and implement appropriate treatment plans. The individual will evaluate and measure the success of the care plan, and document and alter such plans as necessary. The Social Worker facilitates after care plans through research of appropriate resources, and acts as the liaison between patients, patients’ families, and nursing homes to secure placement for patients. The incumbent will accompany involuntary emergency patients to court hearings within the hospital. In addition, the individual will lead educational, discharge planning, and supportive psychotherapy groups and perform other tasks as assigned.

Qualifications:
- Must be an LICSW and Master’s Prepared.
- Must have 3+ years of Social Work experience either within the geriatric or psychiatric arenas.
- Experience working in an acute care environment is preferred.
- Must have excellent communication, problem solving and organizational skills, and must be compassionate and a self-starter.
