* Conduct field visits to perform Assessments with Health Plan members.
* Obtain a thorough and accurate member history including clinical, social, and environmental factors.
* Assess member's biopsychosocial factors.
* Identify need for assistive devices/adaptive equipment for members.
* Assess each member's clinical, behavioral, social, and environmental information to develop Individual Service Plans and a member support system.
* Acts as an advocate for an individual's health care needs.
* Identify members that would benefit from alternative level of care or other waiver programs.
* Establishes short and long term goals in collaboration with the member that meet the member's needs and the referral source's requirements.
* Communicate care objectives to appropriate individuals/departments/referral sources.
* Identify members with potential for high risk complications and coordinate appropriate treatment in conjunction with the member and the health care team.
* Establishes working relationships with referral sources and community resources.
* Review benefit systems and conduct cost benefit analysis.
* Evaluate quality of necessary medical services.
* Acquire data and evaluate necessary health services for cost containment.
* Document effectiveness of service coordination services.
* Collaboratively works with other clinical team members and other departments.
Required:
Bachelors degree from an accredited college or university in nursing, social work, counseling special education or a closely related field or State Waiver.
Preferred:
Masters degree from an accredited college or university in nursing, social work, counseling special education or a closely related field.
Years and Type of Experience:
Required:
Minimum one year of experience working with disabled and elderly individuals in a Service Coordinator, Case Management, or Advocate role.
Preferred:
* One to two years working in the community with community agencies.
* Three years experience in health care, case management, discharge planning, or behavioral health.
Specific Technical Skills:
Required:
Excellent verbal and written communications skills.
* Strong decision making skills.
* Ability to provide services in an environment that involves multiple health care systems.
* Ability to interact with all relevant components of the health care system.
* Ability to provide services that deal with the individual's broad spectrum of needs.
* Self-starter with ability to handle multiple projects at one time.
* Appreciation of cultural diversity and sensitivity towards target population.
Preferred:
* Working knowledge of state's Elderly/Disabled Medicaid program.
* Proficient computer skills.
* Bilingual (ability to read, write, and speak).
Certifications or Licensure:
Preferred:
Current State license to practice Nursing, Social Work or Behavioral Health.
Required:
Must possess a valid driver's license and access to a motor vehicle.
PHYSICAL REQUIREMENTS
* Must be able to operate a computer.
* Must be able to operate (and communicate via) a telephone.
* Must be able to sit for long periods of time.
* Must be able to travel locally to member's homes and provider facilities.
* Must be able to ask questions and understand responses.
