Celebrate Caring at Triad Care and Rehabilitation Center, an affiliated center of SunBridge Healthcare and a leader in long term and rehabilitative healthcare.  We are currently seeking full time Social Services Assistant at our 191- bed center located in High Point, NC.  Long term care experience preferred.

Primary responsibility is to assist the Social Services Department under the direction and supervision of the Director of Social Services or Social Services Consultant in accordance with current guidelines and regulations; and established policies and procedures.  The focus is to assist each resident and family in adjusting to placement, illness, and plan of care to attain the highest level of functioning.

* Strong interpersonal skills.

* Ability to make independent decisions and to follow instructions.

* Ability to manage multiple tasks and projects.

* Ability to participate in and lead Interdisciplinary Teams.

* A related degree and/or experience that fulfills state requirements (in applicable states) for Social Service Designee; Social Work certification/license in accordance with state and local standards.
