Social Worker / 1575
Position Description:
Social Workers are responsible for identifying and providing resources and social supports to members. ESSENTIAL FUNCTIONS: • Responsible for providing support for Nurses in identifying resources for members involved in the disease management program; • Responsible for communicating with community providers in order to access social resources that will improve members’ quality of life including job supports, financial supports, transportation and living supports such as food and clothing; • Performing one on one outreach to members in consultation with their assigned nurse to provide additional information about available services and supports; • Will assist nurses in identifying other areas of the member’s life that may require support and/or behavioral change; and • Will make referrals for members, as applicable.
MINIMUM QUALIFICATIONS: • Bachelor’s Degree in clinical discipline, required. Preference for Master’s Degree and licensure. • Minimum 2-3 years experience in acute care and/or outpatient clinical settings, including experience in coordinating care for consumers with intensive medical or behavioral health needs; • Background and experience in health care targeted by the GAMMP program as well as community systems and resources, case management; • Strong verbal and written communication skills; • Customer service focus; and • Ability to resolve problems in a non-threatening manner.
