Qualifications

- BS or MSW Degree required from accredited educational institution.
- Licensed in the State of Maryland as a LCSW or LCSW-C.
- Five (5) years experience in social work with an emphasis in acute care and/or oncology.
- Ability to communicate and possess strong organization skills

 Responsibilities

- Demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients on unit.
- Develop professional knowledge base.
- Participate in care coordination rounds.
- Demonstrate awareness of patient rights including confidentiality, privacy, informed consent, surrogate decision making and advance directives.
- Counsel patients and families around the issues of emotional adjustment to illness, disability, hospitalization and/or discharge plan.
- Demonstrate the knowledge and skills necessary to determine age specific needs.
- Provide information and referral services to patients and families.
- Demonstrate working knowledge of community resources.
- Interact with patients, families, and health care team to develop proactive plans for continued care and discharge planning.
- Document assessment and interventions in the medical record.
- Provide support for crisis intervention, bereavement counseling.
- Intervene when social/economic issues affect health care and disposition.
